1) To book your Telemedicine appointment, please click the Book Appointment button at the end of this page.

2) Enter your legal name or the person’s legal name whom you are making the appointment for and all other contact information requested.

3) Agree to the Privacy Policy in order to move to the next step

4) Choose the “Book Telemedicine Appointment” button under Appointment Type. If using a mobile phone, click the “Schedule an appointment” button, then choose “Book Telemedicine Appointment” button

5) Choose any available location

6) Choose any available provider

7) Choose available day and time that works for you

8) Confirm your appointment
